Healthy Start Family Support Worker

We are a busy non-profit, Federally Qualified Community Health Center. Primary responsibilities include but are not limited to:

  • Answers telephones, schedules appointments, relay messages in a professional and timely manner to other Healthy Start staff.
  • Assist with purchase order follow- up and monitoring Healthy Start inventory.   Maintain an inventory tracker and monitor counts for office supplies, educational materials, Pac-N-Plays, Car seats etc., stored in office and outside storage unit.
  • Under supervision by staff, complete Initial Assessments with new clients, via face-to-face telephone video calling or home visiting, as indicated by client.  Provide assistance to clients with accessing Community Services and understanding the services provided by our Healthy Start program.
  • Complete all training requirements, as indicated by Healthy Start Standards and Guidelines.
  • Maintain positive working relationships with other Community Agencies and Medical Providers.
  • Compile and maintain stores of initial “New Client” packets for Care Coordinators to use.  Assist with getting all consents and required paper work signed by new clients, during Initial Assessment.
  • Attend all unit meetings, taking meeting minutes as indicated.
  • Participate in all Community outreach activities with providers and other community organizations.
  • Attend all required Healthy Start/Coalition meetings; program updates/documentation changes, educational requirements, etc., via zoom meetings or in-person. (Monthly or Quarterly as indicated).
  • Maintain proficiency in understanding and completing documentation requirements in the Well Family System. (See Standards and Guidelines)
  • Assist with completion of required Quality Assurance activities, as designated by Healthy Start Supervisor.
  • Other duties as assigned.

Qualifications

  •  Education/Experience Associates Degree required, preferable in human services-related field.
  •  Two years’ experience working with families preferred.
  •    Valid Florida driver’s license and insurable by agency’s current insurance carrier.
